What is Hybrid Health Record (HHR)?
A Hybrid Health Record (HHR) is a patient’s record that contains both paper-based records and Electronic Health Records (EHRs). An HHR puts forth the whole picture of a patient’s medical history to providers, as well as has the ability to share information between different care settings. The use of an HHR can help improve communication among providers and patients and reduce duplicate testing and mistakes.
